My Vaccine Pass is an official record of your COVID-19 vaccination status for use within Aotearoa New Zealand. It will allow you to access certain events and venues operating under the COVID-19 Protection Framework (traffic light system).

Domestic digital vaccination certificate
Everyone in Aotearoa is now able to access a digital vaccination certificate. This will include a QR code, which you can print or save to your phone.
You may be required to show this as proof of vaccination in a range of public settings. This could include:
- faith-based gatherings.
Requiring vaccination certificates will be optional for many locations.
You will not be required to show proof of vaccination to access supermarkets, pharmacies, health services, food banks and petrol stations.
There will be options available for people who do not have a smartphone or access to a computer.
Who can get a My Vaccine Pass
You can request a My Vaccine Pass if you are aged 12 or over and:
- have had 2 doses of the Pfizer or AstraZeneca COVID-19 vaccine in New Zealand, or
- been vaccinated overseas with an approved vaccine, or
- have a medical exemption.
Your My Vaccine Pass will include your name, date of birth and a QR code. You can save it on a digital device, like your phone, or print a physical copy.
Your pass will have an expiry date 6 months after the date of issue.
Who does not need a My Vaccine Pass
People under the age of 12 years and 3 months are not required to show proof of vaccination.
If you are unable to get the COVID-19 vaccine for medical reasons, there is an exemption process to get your My Vaccine Pass.
You will need to talk to your doctor and they will apply on your behalf.
International digital vaccination certificate
From late November 2021 you will be able to download a digital certificate as proof of vaccination for international travel.
You can choose to print this and carry it in your passport or save it on your phone or laptop. These certificates will meet international standards.
If you need proof of vaccination before the end of November, you can request a letter from Ministry of Health which confirms your vaccination status — this takes up to 10 working days.
How to request proof of your vaccination
If you cannot access My Covid Record, or if you are travelling before vaccination certificates are available, you can request a vaccination letter from the Ministry of Health.
We recommend waiting until you have had 2 doses before requesting a vaccination letter. If you request a letter after your first vaccination, it will only show proof of 1 vaccination.
How we record your vaccination
When you get vaccinated, we record the details in the national COVID-19 Immunisation Register (CIR).
- your name, birth date, gender and National Health Index (NHI) number
- your address and contact details
- details about the vaccination/s you receive
- any reactions you have to the vaccine.
The CIR will also record if you decide not to get a COVID-19 vaccine.
